Abstract: The British Gas plc Research and Development Division has, for many years, provided a service to Regions requiring assistance following any kind of incident or accident.

This paper details the origins, development and operation of the Engineering Research Station distribution incident investigation service including typical incident involvement, call-out procedures, investigation equipment carried, site investigation techniques, collection of evidence, overall assessment, preparation of final report and possible involvement in litigation.

To date, ERS has been involved in the investigation of more than 100 incidents , predominantly fractures of cast iron pipe remote from any joint or fitting. The most common factors contributing to the failure of cast iron pipes are briefly reviewed.
